Completes evaluations for referred individuals, utilizes appropriate tests and measures, establishing a plan of care in compliance with departmental standards and federal and state regulations. Completes patient treatments efficiently in accordance in accordance with plan of care. Completes accurate charging of services and provides documentation to support the charges and provision of the therapy services within guidelines. Manages each patients plan of care effectively by timely interdisciplinary collaboration and communication with physician involvement as needed. Effectively plans transition of patients within or outside the Hillcrest system.What should I have to be considered? Graduate from an accredited speech therapy program required. At least two years of experience performing as a Speech Therapist in a long term care, post-acute, hospital, home care or similar setting preferred.
